Not medical adviceAlzheimer's Companion is an educational synthesis of published literature for study, patient understanding and family caregivers. It does not replace a clinician. Doses and guideline statements are reported as published, not as instructions for any individual.

Part 3 · Treatment

Cholinesterase inhibitors and memantine

The older Alzheimer's drugs act on the synapse, the gap where one neuron signals the next. Watch one cholinergic signal from start to finish, then switch to Alzheimer's or to a drug and play it again.

Show

Schematic, not to scale. The chart counts the molecules in the drawing; it is not measured data.

Memantine at the NMDA receptor

Memantine works on a different system: the NMDA receptor, a calcium channel opened by the transmitter glutamate. The idea behind it is to quiet a constant low-level leak through the channel while still letting a real signal through.

Blocker

Schematic. The channel has four subunits in reality; two are drawn. Traces are illustrative, not recordings.

How big is the benefit?

Average effects from Cochrane reviews and trials, drawn on the full length of each rating scale so you can see how much of the scale they cover. Then the side effects the labels list, and what happened when people stopped.

Average differences from drug versus placebo. Bars start at zero on each scale.

Published label information

What the US prescribing information (FDA labels) says about starting and raising doses. This is reported as published, for study. It is not dosing advice for any person; dosing is decided with a prescriber.

MedicineLabelled stageLabel titration (as published)Label notes
DonepezilMild, moderate and severe5 mg once daily in the evening; 10 mg only after 4 to 6 weeks on 5 mg. The 23 mg dose (moderate to severe only) only after at least 3 months on 10 mg. 157Half-life about 70 hours; steady state within 15 days. Severe-stage indication added 13 Oct 2006. 119,157
Rivastigmine capsulesMild to moderate1.5 mg twice daily with meals, rising through 3, 4.5 and 6 mg twice daily, at least 2 weeks per step; maximum 12 mg/day. 167If doses are missed for more than 3 days, the label says to restart at the lowest dose. 160,167
Rivastigmine patchMild, moderate and severe4.6 mg/24 h, then 9.5 mg/24 h after at least 4 weeks (the minimum effective dose), then up to 13.3 mg/24 h after at least another 4 weeks. 160One patch at a time, replaced every 24 hours; do not reuse a site within 14 days. Medication errors, mostly extra patches left on, caused hospitalisation and rarely death. 160
Galantamine ERMild to moderate8 mg/day with food, 16 mg/day after at least 4 weeks, optionally 24 mg/day after another 4 weeks. 161Maximum 16 mg/day in moderate hepatic or renal impairment. Not recommended in MCI: 13 deaths versus 1 in two 2-year MCI trials (the label notes the placebo death rate was unusually low). 161,326
Memantine IRModerate to severe5 mg once daily, rising in weekly 5 mg steps to 20 mg/day (10 mg twice daily). 163Target 5 mg twice daily in severe renal impairment. 163
Memantine ERModerate to severe7 mg once daily, rising in weekly 7 mg steps to 28 mg once daily. 16214 mg in severe renal impairment; half-life about 60 to 80 hours. 162
Memantine ER + donepezilModerate to severeFor people already stable on donepezil 10 mg. 164Approved on bioequivalence with the two drugs taken separately. 164

Reading the evidence

Alzheimer's Companion is an educational synthesis of the published literature on Alzheimer's disease, made for health-science students, people living with the disease, and the families who care for them.

It is not medical advice. It cannot diagnose, and nothing here should change a treatment without your own clinician. Guideline doses are quoted as published. Please check the original sources, which are listed in full in the Sources module.

Part of Medical Companions, animated and cited guides to one disease each. Also there: Parkinson's Companion, a related guide to Parkinson's disease, the other common neurodegenerative disease, and FMF Companion, on Familial Mediterranean Fever.
