Gout is curable in one precise sense: keep serum urate low for long enough and the urate crystals dissolve. This module shows what that takes, where the new drugs intercept the urate pathway and how strong the evidence is for each, and why the cure so rarely reaches people. Every route carries an evidence label.
1 · What "cure" means in gout
Gout duration
Schematic curves. The anchors are real: in 18 joints, crystals cleared in 3 to 33 months, longer with longer disease, and the median count fell from 7.5 to 3 per field within 3 months (Pascual 2007). The shape between them is drawn.
Remission: how often, and how soon
Show
Published points only; lines join them. The cohorts used the simplified definition (12 months with no flares, no tophi and urate below 6 mg/dL) but differ in who was enrolled, so compare shapes, not heights.
2 · Where new drugs intercept the pathway
Route
Schematic pathway, not to scale.

Status
Only three routes remove urate. Production, excretion and breakdown lower the urate pool, so they can dissolve crystals. NLRP3 and IL-1 blockers treat or prevent flares; they do not remove crystals. 84,85,250
3 · The implementation gap
Population
Each square is 1% of the cohort. The two panels come from different studies and measures, labelled on each.
Open questions
These are gaps the literature itself leaves open. Each one is a place where a study could change the picture.
